On June 13, 2011, Catalyst Rx completed the acquisition of your pharmacy benefit manager, Walgreens Health Initiatives, Inc (WHI). Beginning in September 2011, you can expect to see references to the WHI name and logo replaced by those of Catalyst Rx.

Please note that you should continue to use your existing member identification card to fill prescriptions. All information on your card, including addresses and phone numbers, will remain valid.

Union Health Service has implemented an automated telephone system for prescription refills. You do not need to speak to a pharmacist or pharmacy staff member to refill your prescriptions. You have two options.

1. Call Union Health Service switch board at 312-423-4200, select language and select option 4 (follow the prompted messages)

2. From your computer
a. go to website www.unionhealth.org
b. Click the pharmacy link on the main page
c. Click Online Prescription Refill Request
d. Enter your prescription information as indicated and submit.
